§REASONS FOR MISSING TEETH

1 If cavity in a tooth is not treated in time , then tooth will go on breaking , and ultimately needs to be removed .

§ STAGES OF CAVITY  AND ITS  TREATMENT

A -- EARLY STAGE --Usually cavity  is present in enamel or shallow in dentine .
TREATMENT -- FILLING

B-- CAVITY IN DENTINE
TREATMENT --FILLING

C-- CAVITY REACHING PULP
TREATMENT -- ROOT CANAL THERAPY

§
D- LATE --Badly broken down tooth

TREATMENT-- ROOT CANAL THERAPY with POST and  CORE AND CROWN

E - VERY LATE -- Tooth irreparably  damaged
TREATMENT -- EXTRACTION  and REPLACEMENT

REASONS FOR MISSING TEETH

2 Plaque and tartar destroy gums and bone tooth starts shaking  which ultimately needs to be removed .

3 FRACTURED tooth needs to be removed .

4 TOOTH MISSING -- IMPACTED or congenitally missing .

§CONSEQUENCES OF TOOTH EXTRACTION

A. When posterior teeth [ molar , premolar ]are extracted biting efficiency reduces .
1. After extraction of a tooth adjacent teeth need to take the load of missing tooth.
2. When third molar is missing biting efficiency does not reduced .
3. When 2nd and 3rd molar are missing , 1st molar is only grinder tooth , biting efficiency reduces by 20%

B. When any tooth is removed , bone starts decreasing in height and width .Bone is a main seat for artificial dentures .

C. When  anterior teeth [incisors , canine ] are extracted
1. Smile is affected .
2. Speech is affected .
3. Face gives older appearance .

§
D. When both adjacent teeth are removed , tooth is  not having any contact to share his load .
This tooth becomes mobile because of hyper function after some time .

E. Teeth have natural tendency to shift in spaces created by extracted teeth .
Teeth adjacent to extraction space becomes tilted , opposing tooth supra erupts , and try to close down spaces .
This movement of teeth causes
1. Increased chances of cavity formation in tilted teeth due to loss of good natural contacts.
2. After few years replacement becomes impossible .
3. Loss of biting efficiency .

C. OVER DENTURE.

§CONSIDERATIONS BEFORE DECIDING TREATMENT PLAN .

HOW TO CHOOSE TREATMENT PLAN ?

DIFFERENT TREATMENT OPTIONS ARE GIVEN BY THE  DENTIST

ADVANTAGES - DISADVANTAGES OF EACH ARE EXPLAINED .

CONSEQUENCES OF NOT TAKING TREATMENT OR POSTPONING ARE EXPLAINED .

NOW PATIENT IS THE BEST JUDGE TO TAKE DECISION .

§CONSIDERATIONS BEFORE DECIDING TREATMENT PLAN .

1. Patients age .

2. Condition of remaining teeth .

3. Time span patient is missing his teeth

4. Patients attitude .

5. Socioeconomic status of patient.

§CONSIDERATIONS BEFORE DECIDING TREATMENT PLAN .

1. PATIENT’S  AGE.

§ Young patient needs more support for longer period . So the best treatment  plan with the best materials should be considered .  Choosing the best always gives maximum benefits for long term .

§At  older age loss occurred is usually more , treatment plan should consider, harmony  between  replaced teeth and remaining teeth . So choosing effective treatment increases life of remaining teeth.

§

2. CONDITION OF REMAINING TEETH

§This is the most important factor which decides the treatment plan . Ultimate success depends on this factor .
§
§Dentist will determine the conditions  and evaluate for proper treatment plan .

§Sometimes it is  possible to improve the conditions of remaining teeth by treatment .
§
§When teeth are missing other teeth are loaded more . Over loading will cause , fatigue of over loaded teeth , ending in loosing of teeth .

§

3. TIME SPAN OF MISSING TEETH

§When teeth are removed , bone will start reducing in height and width . Bone is the main seat for dentures . So preserving teeth is always advisable .
§
§Drifting of teeth in extraction spaces complicate the treatment plan . So replacing of extracted  teeth at the earliest after healing is advisable .

§

4. PATIENT’S ATTITUDE

§Teeth are lost because of negligence of  oral hygiene in the past . Patient should be motivated to maintain good oral hygiene to prevent future problems .
§
§Patient should under stand importance of early dental treatment. EARLY DENTAL TREATMENT is a GOOD INVESTMENT  in terms of

1. MONEY : money involved is always less when treatment is taken at an   early stage, since problem is less .  E.g. cost of R.C.T. is less than IMPLANT OR BRIDGE , FILLINGS EVEN LESS

§
2. TIME : Time involved is less when treatment is taken at an early stage .  So less money . Many a times treatment is postponed not for  money but busy  scheduled of the patient .

3. HEALTH : You remain in healthy condition if you take early treatment . Most of the dental problems are irreversible . Problem will always go on increasing. Dental problems are INFECTIOUS in nature . In fact if you have dental  problem , you are moving with infection in your mouth . Sometimes treatment is postponed because of fear of pain. Today we give  painless treatment , “PATIENT COMES WITH PAIN AND WE RELIEVE HIS PAIN.”

§

5. SOCIOECONOMIC STATUS

§When  patient is involved in social activities or need to meet many people , cosmetic treatments are preferred .
§
§When patients are spending money , it is for their good health and not for luxury.
§
§Ideally choose the best material option since it will be placed in the mouth . They cost more because , they are definitely that worth putting in your mouth .
